Medical Records Request
Patients requesting medical records from Beaufort Memorial Hospital are required to submit a written authorization request form directly to the Health Information Services (HIS) department in person or through the mail the authorization to release medical records form. Please bring identification with you to the hospital when you submit your authorization form. If you are submitting the request by mail please send a copy of your picture identification along with your request. (Examples: Drivers license, identification card, student identification, passport, military identification card). Learn more about requesting medical records from BMH.

Universal Medication Form (UMF)
